Anyone know what is actually wrong with the eye? Its pretty bad looking. Not to cause a panic or nothing. Did he run into something or did it just kinda festered up? Just want to make sure it doesnt happen again if its preventable.

99.5% of the time it is a water quality issue.
A few times with WC fish it can be a bacteria thing but even then I think it is water quality that sets it off.
99% of the time it is easy to fix....water changes and a little salt.
